Requirement: An individual must: need for assistance with Instrumental Activities of Daily Living (IADLs) which include nonpersonal care tasks; and . have a functional limitation
Primary Home Care is a medically related personal care service provided by a physician as part of a client’s plan of care. PHC is available to eligible Medicaid clients whose health problems cause them to be functionally limited in performing activities of daily living. A Primary Home Care attendant provides each client’s services.
Family Care is a service that is provided for eligible aged or disabled adults who are functionally limited in performing daily activities. This program is block grant funded. A person able to have a family-like relationship with the client may provide services.

Community Attendant Services is a medically related personal care service prescribed by a physician as part of a client’s plan of care. CAS is available to eligible Medicaid clients whose health problems cause them to be functionally limited in performing activities of daily living. A Primary Home Care attendant provides each client’s services.
You are eligible to receive our services if you are currently being treated for an illness or have been recently hospitalized and are predominately home-bound. A plan of care will be developed for each patient and will be obtained from the patient’s physician and legally authorized to live in USA
*Clients who do not qualify for services under the Medicaid program may choose to pay for their services out of pocket.
